Assessment of left atrial structural remodeling in patients with cryptogenic stroke - lessons learned from LGE-MRI

چکیده
Background Cryptogenic embolic strokes of undetermined source (ESUS) are thought to comprise about 25% of all ischemic strokes. Late-Gadolinium MRI (LGE-MRI) allows detection and quantification of left atrial structural remodeling (LA-SRM). We sought to compare the degree of LA-SRM using LGE-MRI in patients with ESUS and in patients with embolic stroke of know origin, especially in those with atrial fibrillation (AFIB).

Atrial fibrillation ablation outcome is predicted by left atrial remodeling on MRI.
BACKGROUND Although catheter ablation therapy for atrial fibrillation (AF) is becoming more common, results vary widely, and patient selection criteria remain poorly defined. We hypothesized that late gadolinium enhancement MRI (LGE-MRI) can identify left atrial (LA) wall structural remodeling (SRM) and stratify patients who are likely or not to benefit from ablation therapy. METHODS AND RESU...
متن کاملIncidental MRI findings of left ventricular myocardial scar in atrial fibrillation patients is associated with increased stroke risk
Background Left ventricular late gadolinium enhancement (LV-LGE) is a known incidental cardiac magnetic resonance (CMR) finding in atrial fibrillation (AF) patients and has recently been associated with increased mortality [1]. While LV-LGE is an indicator of poor prognosis in patients with cardiomyopathy, data on the clinical significance of incidental findings of LV-LGE in AF patients is stil...
متن کاملCardiac magnetic resonance imaging: a new tool to identify cardioaortic sources in ischaemic stroke.
Stroke of undetermined aetiology or 'cryptogenic' stroke accounts for 30-40% of ischaemic strokes despite extensive diagnostic evaluation. The role and yield of cardiac imaging is controversial. Cardiac MRI (CMR) has been used for cardiac disorders, but its use in cryptogenic stroke is not well established. We reviewed the literature (randomised trials, exploratory comparative studies and case ...
متن کاملLeft atrial remodeling by MRI: comparison in patients with and without cardiovascular disease
Background Pre-existent scar in the left atrium (LA) is hypothesized to be the arrhythmic substrate of atrial fibrillation (AF) [1,2], and has been characterized in cardiac MR studies [3] using high resolution late gadolinium enhancement (LGE) [4]. Recent pathology studies show that LA fibrosis extent is similar in patients with AF vs. other cardiovascular disease (CVD) [5], e.g. valvular disea...

Letter by Kosiuk et al Regarding Article, “Implication of Left Ventricular Diastolic Dysfunction in Cryptogenic Ischemic Stroke” To the Editor: We read with great interest the article by Seo et al that adds new evidence to the discussion on the link between left ventricular diastolic dysfunction (LVDD) and ischemic stroke. In their work on patients with strokes of different origin, including st...
